What will your Connectus medical home team do for you?

1. Your healthcare provider and support team will answer questions you have about your health and how to better your health.

2. If you need help from other healthcare providers (not at Connectus Health), your healthcare team will help you connect with those other providers (like a specialist).

3. Your healthcare provider and support team will help you set healthcare goals that you choose and are important to you.

4. Your healthcare provider and support team will help you with your healthcare needs but can also assist with other needs like free food items, free diapers, medication help, and helping you get to and from your appointments.

5. You will have access to a team member 24 hours a day, 7 days a week for any questions/concerns that you may have when the clinic is closed at night and over the weekend.

How can you help the Connectus team provide the best care and support for you?

1. Come to all your appointments and do your best to be on time for all your appointments.

2. Work with your Connectus team to better your health. Ask questions and let the team know when you don’t understand something.

3. Tell the Connectus team when something is not working (such as you’re having trouble with medication or you’re not able to check your blood sugar as often as you told your provider you would).

4. Reach out to your Connectus team for help first (unless it’s a medical emergency) instead of going to the emergency room or hospital.

5. Let us know how you feel about your care team and the care and help you’re getting at Connectus.

How can you talk to your healthcare team when the clinic is closed?

If you’re experiencing a medical emergency, please call 911. If you’re not experiencing a medical emergency and have questions that can’t wait until the clinic reopens, please call 615-292-9770 and follow the instructions for contacting the on-call provider. Someone will answer the phone and ask you some questions. Your information will be given to a provider on our team and the provider will call you back as soon as possible.

This means that we are a team and we will work together to better your health. When you choose Connectus as your medical home, YOU become the most important member of your healthcare team and YOU are in charge of your health. Your Connectus medical home team will include your healthcare provider, other team members who may support you, and most importantly - YOU!
