Connectus Health is a non-profit community healthcare organization founded on the values of Compassion, Wellness, Community, and Culturally Sensitive Care.
Our staff and facilities help manage the healthcare of the entire family and community, including those who do not have health insurance or are underinsured.
At Connectus Health, we are dedicated to providing affordable, high-quality, evidence-based healthcare that is designed around you and your healthcare needs. Connectus utilizes the Patient-Centered Medical Home (PCMH) model of care. A Patient-Centered Medical Home allows you to personalize your care plans, supports better understanding and monitoring of prescription medications, provides tailored healthcare coaching and advice, and creates stronger relationships with your healthcare team and other community resources.
Payment Assistance
For the Uninsured/Underinsured, we offer medical care on a sliding fee scale. All patients are asked to complete a sliding fee application based on income and household size. This determines the visit cost.
TennCare, Cover Kids and Medicare are accepted at Connectus locations. Most private health insurance plans are accepted, and if you need assistance with securing insurance, we can help. Connectus Health is also available to provide support with getting connected to other community resources for those with financial, housing, or employment needs.
